Join ITAV

Our Membership Agreement asks for:

1. Name, Address, Home Phone, Cell Phone, Email and Date of Birth for each member of the household who is joining.
2. Name, Address, Home Phone, Cell Phone, Email and Relationship of 2 Emergency Contacts.

The agreement has the following provisions:

1. ITAV provides services to its members through its volunteers. All of its volunteers undergo a background check.
2. Services are provided between 9:15 and 4:45  Monday through Friday. Members must make requests for service at least 2 full business days before the date of the service by calling or emailing ITAV. If a ride is needed on Tuesday, the request must be made by the prior Thursday. It is possible that ITAV will not be able to provide the service requested.
3. The membership fee is $25 per household.
4. ITAV reserves the right to terminate the agreement at any time that it determines that it is in the best interest of ITAV, its volunteers, other Members or the undersigned member. Members may terminate the agreement at any time by providing written notice  to ITAV.
5. ITAV will take all reasonable steps to protect the personal information of its Members.
6. The Member indemnifies and agrees to hold ITAV harmless for any loss, expense or liability arising out of the activities of its employees, board members , or volunteers. As a member of ITAV, the member understands that ITAV is not a provider of emergency services or health- care services and that ITAV does not employ licensed health-care  professionals or social worker.
